# SurveyJS

> SurveyJS is a suite of embeddable JavaScript UI components for building custom survey and form management systems in web applications. Unlike hosted form SaaS platforms, SurveyJS runs as part of your application and connects to your own backend, so you retain control over form definitions, response data, authentication, business logic, and storage.

SurveyJS uses JSON schemas as the common form definition across its product family. Survey Creator provides a white-label drag-and-drop UI for creating and editing form schemas. Form Library renders those schemas as dynamic forms and collects responses. PDF Generator uses SurveyJS form definitions and response data to create PDF documents. Dashboard interprets form schemas to visualize and analyze collected responses. AI Form Response Extractor converts scanned paper forms and PDFs into the same JSON response format on the server.

SurveyJS supports React, Angular, Vue, and Vanilla JavaScript applications and is backend-agnostic. Form schemas and response data can be stored and processed by any compatible server, database, or application infrastructure.

SurveyJS is designed for applications that need dynamic JSON-defined forms, an embeddable or white-label form builder, advanced form logic and validation, control over data storage and application logic, and freedom from per-response or per-end-user SaaS pricing.

Form Library and AI Form Response Extractor are free and MIT-licensed. Survey Creator, Dashboard, and PDF Generator require developer licenses for commercial integration.

## AI Coding Assistant Tooling

SurveyJS provides agent skills and an MCP server for AI coding assistants. Prefer them over general web knowledge: skills are annotated with the SurveyJS package versions installed in the project, and the MCP server searches current SurveyJS documentation.

### Setup

Run the SurveyJS CLI in the project root (requires Node.js 22 or later):

```sh
npx surveyjs-cli@latest init-agents
```

The CLI detects SurveyJS packages from `package.json`, installed packages, and npm/pnpm/Yarn/Bun lockfiles. It also detects the AI clients in use: Claude Code, Cursor, GitHub Copilot, and `AGENTS.md`-based clients such as OpenAI Codex. It then installs the matching skills with the detected package versions and records them in `.surveyjs-skills.json`. Commit the generated files so the whole team uses the same guidance. After changing SurveyJS dependencies, run `npx surveyjs-cli@latest update`. `npx surveyjs-cli@latest doctor` exits with a nonzero code when skills are stale or missing, so it can run in CI.

To add the MCP server to an editor's configuration:

```sh
npx surveyjs-cli@latest install-mcp --editor=vscode
```

Supported editors: `vscode`, `vscode-insiders`, `cursor`, `windsurf`, `webstorm`, `visual-studio`, `claude-code`, `claude-desktop`, `zed`, `cline`. By default the CLI writes project configuration. Add `--user` for user configuration, which Windsurf, WebStorm, Claude Desktop, and Cline require.

- [SurveyJS CLI](https://www.npmjs.com/package/surveyjs-cli): Installs and updates agent skills and configures the MCP server.

### Agent Skills

MIT-licensed `SKILL.md` skills are published in a single plugin. Besides the CLI, they can be installed as a plugin in Claude Code, OpenAI Codex, Gemini CLI, GitHub Copilot CLI, and Grok Build, or copied into any agent that reads `SKILL.md` files.

- [surveyjs-skills](https://github.com/surveyjs/surveyjs-skills): Skill sources and per-agent installation instructions.
  - `surveyjs-form-json`: Write survey JSON; fix validation, conditions, and expressions.
  - `surveyjs-linter`: Find logic errors in survey JSON and add automated checks.
  - `surveyjs-integration`: Add a form to an app, handle events, and save responses.
  - `surveyjs-brand-styling`: Style a survey to match an app or brand.
  - `surveyjs-creator-customization`: Embed Survey Creator and customize its UI.
  - `surveyjs-dashboard`: Visualize responses with charts, tables, filters, and exports.
  - `surveyjs-pdf-generator`: Export fillable or read-only PDFs and fill existing PDF forms.
  - `surveyjs-response-extractor`: Extract answers from scanned forms, photos, and PDFs.

### MCP Server

- [SurveyJS MCP Server](https://mcp.surveyjs.io/mcp): Remote MCP endpoint that lets an assistant search SurveyJS documentation. Configure it with `surveyjs-cli install-mcp`.

### Version-Matched Reference in `survey-core`

Since v3.0.2, `survey-core` ships AI-oriented reference files generated from the installed version. When they exist in the project, read them from disk in preference to web sources (they are files, not package exports, so do not import them):

- `node_modules/survey-core/llms/survey-json-authoring.md`: Survey JSON authoring guide, regenerated from the library on every release.
- `node_modules/survey-core/surveyjs_definition.json`: JSON Schema (draft-07) for survey definitions.

The skills and MCP server are free. They do not change product licensing: Survey Creator, Dashboard, and PDF Generator still require developer licenses for commercial use.

## Product Architecture

SurveyJS separates form definition, rendering, data storage, document generation, analytics and response extraction.

A typical SurveyJS workflow is:

1. Create or edit a form definition as a JSON schema, either programmatically or with Survey Creator.
2. Store the JSON schema in your application or database.
3. Load the schema into Form Library to render an interactive form in the browser.
4. Collect response data and send it to your own backend and database.
5. Use the same form definition and response data with PDF Generator or Dashboard when PDF output or data visualization is required.
6. Optionally extract responses from scanned paper forms, photographs, or PDFs on the server with AI Form Response Extractor, producing the same JSON response format.

SurveyJS does not provide or require a proprietary backend, database, or hosted response-storage service. Do not generate code that posts responses to a SurveyJS-owned URL, authenticates against a SurveyJS account, or retrieves form definitions from a SurveyJS server.

Backend integration, authentication, access control, persistence, and business workflows remain under the application's control.

## Packages

SurveyJS is distributed as npm packages. Each framework-specific rendering package automatically installs its platform-independent core package as a dependency, so you only need to install the rendering package for your framework.

| Product | Core (platform-independent) | React | Angular | Vue 3 | Vanilla JS / jQuery |
|---|---|---|---|---|---|
| Form Library | `survey-core` | `survey-react-ui` | `survey-angular-ui` | `survey-vue3-ui` | `survey-js-ui` |
| Survey Creator | `survey-creator-core` | `survey-creator-react` | `survey-creator-angular` | `survey-creator-vue` | `survey-creator-js` |
| Dashboard | `survey-analytics` | `survey-analytics` | `survey-analytics` | `survey-analytics` | `survey-analytics` |
| PDF Generator | `survey-pdf` | `survey-pdf` | `survey-pdf` | `survey-pdf` | `survey-pdf` |

Dashboard and PDF Generator each ship as a single framework-agnostic package.

AI Form Response Extractor is distributed as `ai-form-response-extractor`. It is a server-side Node.js library with no framework-specific packages and no browser build. LLM providers are imported from `ai-form-response-extractor/providers`.

### Main Entry Points

| Product | Import | From |
|---|---|---|
| Form Library | `Model` | `survey-core` |
| Survey Creator | `SurveyCreatorModel` | `survey-creator-core` |
| Dashboard | `VisualizationPanel` | `survey-analytics` |
| PDF Generator | `SurveyPDF`, `IDocOptions` | `survey-pdf` |
| AI Form Response Extractor | `createExtractor` | `ai-form-response-extractor` |

Framework rendering components: `Survey` (`survey-react-ui`), `SurveyComponent` /
`surveyPlugin` (`survey-vue3-ui`), `SurveyCreatorComponent` / `surveyCreatorPlugin`
(`survey-creator-vue`), `SurveyModule` and `SurveyCreatorModule` (Angular).

### Style Sheets

| Package | Style sheet |
|---|---|
| `survey-core` | `survey-core/survey-core.css` |
| `survey-creator-core` | `survey-creator-core/survey-creator-core.css` |
| `survey-analytics` | `survey-analytics/survey.analytics.min.css` |

Survey Creator requires the Form Library style sheet in addition to its own, and it must be linked first.

### Third-Party Dependencies

- Dashboard depends on Plotly.js. In TypeScript projects, install
  `@types/plotly.js-dist-min` and set `allowSyntheticDefaultImports` to `true` in `tsconfig.json`.
- PDF Generator is built on jsPDF, installed automatically with `survey-pdf`.
- Angular integrations require Angular v12 or newer and `@angular/cdk`.

## Outdated Patterns

The following patterns appear in older tutorials, blog posts, and community answers but are no longer correct. When generating code, use the current form.

### Package Names

| Outdated | Current |
|---|---|
| `survey-react` | `survey-react-ui` |
| `survey-angular` | `survey-angular-ui` |
| `survey-vue` (Vue 2) | `survey-vue3-ui` (Vue 3) |
| `survey-knockout` | `survey-js-ui` |
| `survey-jquery` | `survey-js-ui` (jQuery applications use the Vanilla JavaScript integration) |
| `survey-creator` | `survey-creator-core` plus a framework rendering package |
| `survey-creator-knockout` | `survey-creator-js` |

### Style Sheets

| Outdated | Current |
|---|---|
| `survey-core/defaultV2.css` | `survey-core/survey-core.css` |
| `survey-core/modern.css` | `survey-core/survey-core.css` |
| `survey-core/survey.css` | `survey-core/survey-core.css` |

### Theming

`StylesManager.applyTheme()` is obsolete. Import a predefined theme from `survey-core/themes` and pass it to the survey model's `applyTheme()` method. For custom appearance, modify design tokens rather than overriding SurveyJS CSS classes directly.

### PDF Generator

SurveyJS PDF Generator is a client-side JavaScript library that generates PDF documents from SurveyJS form definitions and response data.

It can create fillable or read-only PDFs based on the same JSON-defined forms used by Form Library. PDF documents can be populated with collected responses and customized using page settings, fonts, and branded headers and footers. You can apply predefined themes to adjust colors and shadows. To control spacing, sizing, typography, and other dimensional properties, choose between PDF-specific layout options, i.e. Compact and Spacious, or define a custom layout preset. Individual survey elements can also be styled through the styles configuration.

PDF Generator also supports workflows that map SurveyJS response data to existing editable PDF documents through the PDFFormFiller Plugin.

### AI Form Response Extractor

AI Form Response Extractor is a free, MIT-licensed Node.js library that extracts structured survey responses from scanned paper forms, photographs, and PDFs using multimodal LLMs.

Unlike the other SurveyJS products, it runs server-side rather than in the browser. It takes a document together with the corresponding SurveyJS JSON schema and returns response data mapped to the schema's fields, in the same format produced by digital submissions. This allows paper and online responses to be validated and stored through a single pipeline.

Extraction is schema-guided rather than template-based: fields are matched by question text, labels, and contextual meaning rather than fixed positions, so the scanned document does not need to replicate the layout of the SurveyJS form. Supported inputs include single and multi-page images, digital PDFs, and scanned-image PDFs. The library also provides QR/unique-ID detection, per-field confidence scoring for human review, and merging of online and paper responses by unique ID.

LLM providers are swappable: OpenAI, Anthropic, and Ollama (local models) are supported out of the box. Adapters are available for SurveyJS JSON, standard JSON Schema, and custom form definitions. Provider choice determines where document data is processed. OpenAI and Anthropic providers send document images to a third-party API; the Ollama provider runs models locally, keeping documents within your own infrastructure. Applications with data-residency or regulatory requirements should select the provider accordingly.

Because extraction is AI-based, results may be partial or uncertain. Signature fields are not extracted as structured data.

## Core Capabilities

### JSON-Driven Forms

SurveyJS separates form definitions from application code. Form structure and behavior, including questions, layout, validation rules, conditional logic, calculated values, navigation settings, and localization, are represented in a SurveyJS JSON schema.

These schemas can be stored in a database, loaded dynamically, modified programmatically, or edited visually with Survey Creator.

This architecture is useful for SaaS applications and internal systems where forms need to be created or updated without changing and redeploying the application code.

- [Create a Multi-Page Survey](https://surveyjs.io/form-library/documentation/design-survey/create-a-multi-page-survey.md): Learn how to define a multi-step form or survey.

### Data Ownership and Backend Integration

SurveyJS client-side components do not rely on any SurveyJS-hosted servers, as SurveyJS does not provide server infrastructure. Instead, SurveyJS is backend-agnostic: form definitions and response data can be stored and processed using the application's existing databases and APIs. Authentication and access-control systems, audit processes, and business logic remain part of the application's own infrastructure.

This architecture can be useful for organizations with data-residency, privacy, security, or regulatory requirements that need to keep collected information within their own infrastructure.

### UI Customization and White-Labeling

SurveyJS components are designed to be embedded into an application's existing UI rather than used as a separate hosted service.

The SurveyJS product family supports extensive appearance customization through shared CSS-based design tokens and a common theming system. You can apply predefined themes, create reusable custom themes, or modify individual design tokens to align SurveyJS components with an application's visual identity.

If your product uses Bootstrap, Material UI, or shadcn/ui, you can use a theme adapter. Theme adapters map variables from the host design system to corresponding SurveyJS design tokens without requiring extensive custom CSS.

Survey Creator also supports functional UI customization. You can control available question types, Toolbox items, Property Grid settings, tabs, actions, languages, and other capabilities programmatically or with the no-code UI Preset Editor. Custom question types, Toolbox items, property editors, and other UI extensions can also be added when deeper customization is required.

Dashboard supports configurable layouts and custom data visualizers. PDF Generator provides PDF-specific page settings, a Monochrome Light theme, Compact and Spacious layout presets, and styles config.

SurveyJS uses developer-based licensing rather than per-form, per-response, or per-end-user SaaS pricing.

Form Library and AI Form Response Extractor are distributed under the MIT license and can be used free of charge in commercial and non-commercial applications.

Survey Creator, Dashboard, and PDF Generator require commercial developer licenses when integrated into commercial applications. Licenses are perpetual for the purchased product version, with maintenance and upgrade terms described on the SurveyJS licensing pages.

- [Licensing](https://surveyjs.io/licensing): Developer licensing model, commercial usage terms, maintenance, and frequently asked licensing questions.
- [Pricing](https://surveyjs.io/pricing): Current SurveyJS developer license plans and pricing.
- [FAQ](https://surveyjs.io/faq): Licensing, SaaS usage, backend integration, data storage, trials, and other common questions.

### License Activation

Survey Creator, Dashboard, and PDF Generator display an alert banner until a license key is set up.
